INTRODUCTION:

The information requested reflects the percentage of expense line items as related to sales for the period ending December 31, 2009 or the most recent 12 month fiscal period. Please use percentages only. Do not use dollars except where requested.

If you operate more than one sheet plant, you must submit data for each one of your facilities.

A report on the results of the survey will be presented on a de-identified basis and will only be made available to those that participate.

Please complete the data entry no later than March 1, 2010. The individual company information you provide for the de-identified report will be destroyed upon compilation of the report.

FBA Member Logon:

If you are a Member of the Fibre Box Association and have a logon for the members area of www.fibrebox.org, please use that logon to access the online forms. If you are a member but do not have a logon for the members area of www.fibrebox.org, please follow the non-member instructions below.
